Job Description: 

Are you interested in making a different in someone’s life?  UCP of Berkshire County is looking for an AFC Care Manager Supervisor to plan, coordinate and monitor AFC efforts. Duties will include conducting initial and ongoing assessments, participating in the plan of care, safety and emergency requirements, Referrals, Intakes and discharge planning for members; the selection, training, supervision and evaluation of AFC Care Managers and Caregivers. The Supervisor will complete all billing, Quality Assurance and reports on a timely basis. The Supervisor is a non-exempt role and will report to the Director of Program Operations.

Bachelor’s degree and social worker’s license from the Massachusetts Board of Registration in Social Work, plus at least two years recent experience working with elder or disabled adults or a bachelor’s degree and two years clinical experience in the care of elders or disabled persons

  • Prior supervisory experience

  • Carry a caseload and provide monthly on-site visits

  • Highly attuned to the needs of the service population, cultural sensitivity

  • Ability to collect and analyze data, define issues and draw conclusions

  • Comfortable thinking outside the box to meet challenges with innovative solutions

  • Effective listening and communication skills and have technical writing skills

  • Comfortable facilitating meetings and working in interdisciplinary teams

  • Strong understanding of treatment modalities and assessment

  • Ability to participate in all required training and maintain Adult First Aid/CPR certification

  • Interest in professional development

  • Self-motivated to manage work load and prioritize tasks with appropriate urgency

  • Proficient computer skills to use agency computer programs

  • Ability to work flexible hours

  • Valid driver’s license and availability of a private vehicle

  • CORI, SORI and National Fingerprint Check required

  • Pre-employment physical exam and baseline TB test
