Job Title:  Registered Nurse

Department:  Adult Family Care Program

Status:  Part-time 20 hrs per week

Job Description:

The AFC Registered Nurse works with the participant, family and the members of the multidisciplinary professional team to meet the nursing needs, develop a plan of care, provide ongoing support and maintain proper paperwork for each AFC participant.



The registered nurse must be fully licensed by the Massachusetts Board of Registration in Nursing.  The registered nurse must have at least two years’ recent experience in direct care of the elderly or disabled adults.

1.Completing and signing the designated screening tool.
2. Completing the nursing assessments and coordinating all other applicable clinical assessments.
3. Developing and reviewing on an ongoing basis each member’s AFC plan of care.
4. Selecting, training, evaluating and supervising AFC caregivers in conjunction with the care manager.
5. Reviewing the physician summary
6.Monitoring the health status of all members to ensure that all needed AFC is properly delivered.
7. Reporting changes in health status of any member to the Member’s physician.
8. Periodically reviewing AFC caregiver logs.
9. Conducting on-site visits with each member at the qualified setting monthly, bimonthly or more often as the member’s condition warrants.
10. Completing a nursing progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants.
11. Submitting a semiannual health-status report to the member’s primary care physician.
12. Planning for and implementing discharges from the AFC program.
13. Conducting an orientation for each AFC caregiver before the AFC caregiver begins personal care.
14. Providing ongoing training to AFC caregivers on health and aging.
